Opioids come in many shapes and sizes. Are they really all that different or merely variations on a theme? There are some predictable variables in opioid pharmacokinetics, pharmacodynamics and pharmacogenetics. However throw into the mix individual variability in patient response to opioids and things start to get interesting.
This PBLD will use several case examples to explore when and why it makes sense to one opioid or perhaps a combination of opioids over another to treat perioperative pain. The aim is to come away with some practical guidelines for best practice in opioid use.

You are the consultant on call at a busy metropolitan hospital. The registrar calls about a woman in casualty who has just arrived after a motor vehicle collision. Her injuries include a closed head injury, right sided chest injuries and a markedly displaced fractured femur. She is obviously pregnant, but has been unable to state how many weeks.
This session will discuss the pregnant trauma patient, including your initial assessment and resuscitation, problems with investigations, and ongoing management.
There will also be a discussion of the priorities and logistics of caring for both mother and foetus

After completing an orthopaedic list at a small private hospital you are phoned by recovery staff who advise that your last patient is still in considerable pain, despite having had all of the fentanyl you had charted. The PCA was commenced, but is not providing any further benefit. The recovery nurse asks you, "Why aren't the opiates helping her? What do you want me to do for her now?"
This PBLD will provide an overview of neuropathic pain for anaesthetists, including its successful diagnosis and treatment, and when to refer for ongoing pain centre management. Emphasis will be directed towards perioperative neuropathic pain and first line management options most helpful in anaesthetic practice.

A previously healthy 54 year old lady presents 2 years after colonic resection for andenocarcinoma with an hepatic metastasis in the right lobe of the liver. This was detected following surveillance with serial CEA and subsequent abdominal CT and PET scans. We will discuss the provision of anaesthesia for hepatic resection and explore the options of care in the perioperative period.

In this workshop we will outline the new neonatal resuscitation guidelines as developed by ARC in 2011. In addition to these updates we will review the neonatal algorithm for resuscitation and then have time to have practice positive pressure ventilation, CPR, neonatal intubation and UVC placement. We will also touch on the topic of neonatal cooling in hypoxic ischaemic encephalopathy.

PERIOP-SCOPE UP!
A 61 year old Greek man had a recent panendoscopy and you see him in clinic a week before his ultra-low anterior resection for cancer. Apart from insulin-dependent type 2 diabetes, hypertension, obesity, and poor exercise tolerance, you pick up clues to potentially new diagnoses of obstructive sleep apnoea, moderate renal impairment, and silent myocardial ischaemia. Also, past anaesthetics were complicated by prolonged intra and post-operative hypotension. What should be organised for this patient's perioperative evaluation and care? What are the implications of these co-morbidities, and what is his risk? This PBLD will stimulate discussion of our role as perioperative physicians in dealing with this increasingly common patient archetype.

Whilst conscious, the patient remains haemodynamically unstable with a rigid abdomen and FAST ultrasound evidence of free fluid, likely from the liver. Healthcare delivery is by necessity an activity that consumes large amounts of resources and produces considerable quantities of waste. It can be somewhat distressing for the environmentally conscious anaesthetist to see how much waste we throw away at the end of a day's work.
This workshop looks at ways the anaesthetist can minimise the amount of waste produced in the operating theatre and the best ways to segregate the waste produced. Whilst recycling is a common practice at home, it is considerably more difficult in the operating theatre. At this workshop we will examine the recyclability of commonly used products. Robert Sharp, a waste expert from SteriHealth, will explain the ins and outs of recycling operating theatre waste.

However all of us must be able to perform this procedure competently and in a timely fashion, meaningly instantly. These workshops are in two parts. The first part will look at the role of video/optical laryngoscopes and LMA type devices in failed intubation. The second part will look at the rapid performance of a surgical airway- the anaesthetist's airway of last resort. Although a serious subject come and enjoy this fun and interactive workshop in a no-stress atmosphere so you'll be ready for the "real thing"!

Maternal haemorrhage is becoming more common due to the change in mothers' demographics, including later pregnancies and the incidence of placenta previa and accreta. These cases are stressful, require intensive resources, and may develop into multi-organ failure with coagulopathy. This workshop will cover the management issues in both anticipated and unexpected maternal haemorrhage.

Hypnosis provides valuable insights into how anaesthetists can communicate more effectively when patients are so stressed they cannot cooperate with their clinical care. These techniques are used on a daily basis by the facilitator in challenging patients, including those with needle phobia, women in labour, screaming children and patients presenting with excessive anxiety.
This workshop will demonstrate a structured, practical approach to communicating hypnotically with patients and its adaptation to enhancing anaesthesia care. The exploration of language structures, underlying conscious and intuitive (subconscious) aspects of communication can be utilised to improve almost any anaesthesia interaction.
Workshop participants will also be shown how a rapid self hypnosis technique can be taught to patients in less than 3 minutes so they can relax without the need for sedative pharmacological premedication.

"A case I wish I had made up" - unintentional cardiothoracic anaesthetics.
A 60 year old woman is having a laparoscopic repair of an incisional hernia. Her surgical history includes an abdominal hysterectomy complicated by post operative pulmonary edema requiring re-intubation. Her medical history is remarkable for morbid obesity, obstructive sleep apnea, hypertrophic cardiomyopathy, diastolic heart failure and anxiety. Your resident calls you for assistance after one of the access trocars lacerates the diaphragm and tears the left lower lobe of the lung.
This PBLD will raise issues including management of comorbidities with conflicting management priorities, lung isolation options for unexpected complications, and altering fluid and pain management when the case suddenly changes from abdominal to thoracic surgery.

A colleague you've known for a few years is investigated for overuse of unusual item numbers for spirometry and ultrasound. Gradually, more suspicious behaviour is detected such as complications from unnecessary invasive monitoring - infections, arrhythmias, and haematomas from using pulmonary artery catheters for non-cardiac patients. A 3 year old boy presents to ED at 6pm with a purple bead firmly lodged in his ear. It needs to be removed under anaesthesia. He is extremely cranky, fasted since lunchtime at daycare and and pouring snot. How do you approach this case?
Anaesthesia for small children can provide interesting challenges for both the paediatric anaesthetist and the occasional paediatric anaesthetist. This discussion will use a case scenario to focus on common issues facing us when we anaesthetise children, such as fasting and premedication, children with an URTI, choice of method of induction and other pearls that may make the process easier on the anaesthetist and safer for the child.

These interactive workshops on the anaesthesia of zoo and wild animals will be held on-site at Taronga Zoo. The workshop will include a lecture and a series of demonstrations that covers all aspects of chemical restraint of zoo and wild animals. This will include pre-anaesthetic considerations when anaesthetising these animals, equipment and drugs that are used and some of the unique and difficult scenarios that confront the zoo veterinarian. Delegates will have the opportunity to experience darting equipment first hand, have close encounters with some of the animals, and visit one of Sydney's most famous icons. This workshop will cover various aspects of acute pain management in children and attempt to delineate the major differences between pain management in children and that in adults.
Common post -operative scenarios from ENT, general surgery and orthopaedic lists will be used to facilitate discussion. Common techniques and drugs will be discussed together with their indications, contra-indications and specific precautions.
Available strategies and the difficulties involved in providing procedural pain management will also be covered. The contribution of an anaesthetist in the management of chronic pain will also be discussed.

"Little" Al needs a semi-urgent fixation of his jaw injury; but Al is an anxious, obese, 9 year old who is awaiting tonsillectomy for sleep apnoea and has just recovered from an exacerbation of atopic asthma. Malignant hyperthermia is an inherited predisposition to a life-threatening syndrome triggered by commonly used anaesthetic drugs. For a positive outcome early recognition and appropriate treatment are essential. During this workshop a number of different scenarios will be discussed in order to explore the changing presentation and treatment of this clinical syndrome, the diagnostic tests that are available as well as who and where patients should be sent for investigation. Anaesthesia preparation for a known susceptible patient will also be discussed.

Anaesthetists are in an excellent position to take advantage of the increasing availability of high performance portable ultrasound machines. This workshop aims to provide an introduction to transthoracic echocardiography (TTE) and discuss how TTE may assist in perioperative anaesthesia care. It will include "hands on" instruction in scanning and cardiac imaging and demonstrate the anatomy visible in a limited study. The workshop is for those with little or no experience in TTE but does assume some basic familiarity with ultrasound.

See one, do one, teach one. Ever wondered if effective teaching techniques might involve more than this? All trainees at some time or another will be involved in teaching and supervising more junior trainees, residents, interns or medical students and this is usually done with no formal guidance. This is made even more challenging in the busy and hectic environment of the operating or hospital system. This workshop is designed specifically for trainees interested in gaining skills in preparing, supervising, teaching and giving feedback to those who you might want to teach a thing or two.
Hone your skills, test your clinical acumen and responses to real time cardiac emergencies trying to save HARVEY. He is a very obliging full-size manikin who realistically simulates nearly any cardiac disease at the touch of a button by varying blood pressure, pulses, heart sounds and murmurs. It will be an adrenaline pumping ride of a workshop for trainees and the good news is that at the end of the day, HARVEY has a reset button.

Ask any anaesthetist what they fear most and invariably the answer is "can't intubate, can't ventilate" scenario. Less than half of us will ever need to perform an emergency surgical airway. However all of us must be able to perform this procedure competently and in a timely fashion, meaningly instantly. These workshops are in two parts. The first part will look at the role of video/optical laryngoscopes and LMA type devices in failed intubation. The second part will look at the rapid performance of a surgical airway- the anaesthetist's airway of last resort. Although a serious subject come and enjoy this fun and interactive workshop in a no-stress atmosphere so you'll be ready for the "real thing"!

In anaesthesia, bleeding is either expected or unexpected and in either case it adds considerably to one's intraoperative workload.
In this workshop we focus on the unexpected bleeding patient and will discuss three real life scenarios and focus on what happened, what went well, and what we can learn to from these cases.
One is a trauma patient, one is obstetric and one is a "surgical oops."
In all cases, managing the immediate situation is really important, but managing the logistics of a sudden increase in workload is also crucial as is the need for leadership in a crisis situation and we will focus on these two aspects.

A 75 year old man is referred by an orthopaedic surgeon who has recently started working with you. He requires early preoperative evaluation with regard to a knee replacement, in light of his history of ischaemic heart disease treated with coronary artery stents. The surgeon wants your opinion on "should we take him on", and if so, "could you sort out his preparation?" When you meet the patient, his limited mobility and comorbid conditions raise questions about his evaluation, optimal preparation and suitability for the local private hospital versus a tertiary centre.

An obese 36 year old mother presents to A&E following a near-syncopal episode with severe headache and neck stiffness shortly after dinner. Previous history is unremarkable except for mild hypertension. CT shows an anterior communicator aneurysm not suitable for coiling. You are called in at 10pm for emergency clipping, and on the way in you are advised that the level of consciousness is falling. This PBLD will discuss issues including airway management on induction, blood pressure control, mannitol administration, and planning ahead in case of acute intra-operative aneurysm rupture.

A 66-year-old widower who still smokes requires an endoluminal stent of his abdominal aortic aneurysm. He has had coronary stents, does not exercise, and ignores the advice of his cardiologist.
A 50-year-old lady with seemingly mild rheumatoid arthritis has accelerated atherosclerosis. She needs a total hip replacement. You are worried when she reports pain down her left arm of new onset.
In a private hospital the nurse rings you to say your next patient, who has a dangerous airway malignancy, has a pulse of 150bpm.
In considering these patients we will discuss smoking, stents, timing of surgery, pre-operative evaluation, airways, analgesia, and risk and postoperative complications. We will also find evidence and guidelines to help treat complex cardiac patients having non-cardiac major surgery.

Due to an increasing insertion rate for pacemakers and defibrillators, anaesthetists are faced with patients presenting for non-cardiac surgery with these devices in situ.
This workshop will focus on the management of these patients by way of several case presentations. Special emphasis will be made on the use of the magnet and the dangers of electrocautery.

This whole day workshop is designed for anaesthetists wanting an update on the current management of trauma, including the bleeding laparotomy and the expanding area of neurointerventional procedures. There will be several scenarios on clinical skills, teamwork and crisis management, all using simulation teaching and debriefing by an expert faculty. This will be a worthwhile refresher for anyone who might be called to manage the emergency trauma case - prepare for the real thing by experiencing the stress and pressure but in a teaching, high fidelty simulation environment!

To update and finesse practice for routine after-hours obstetric anaesthesia, and to manage emergency situations arising in pregnant patients. The course applies generic principles of crisis management and evidence-based clinical practice guidelines to a range of common and rare situations specific to the obstetric patient.
